Case Management Nurse Liaison. The inpatient case management model begins at the acute care hospital. Liaisons begin the patient record before the patient is actually admitted into the system. This enables them to build a case record and also gives the nursing staff valuable information after admission. Recommendations Through the normal course of patient care, therapist and nurses make recommendation for post discharge. These include DME, Follow-up and Supervision. These recommendations along with their assessments come together into an electronic interdisciplinary rounds document. Rounds Case management, as the director of rounds, assembles a weekly document that considers the recommendations of the physicians, therapists, and nursing.
